BCBSAZ Health Choice Pathway (HMO D-SNP) values your input as a member. If you receive a survey, we hope you’ll give your feedback.
Our goal is to give you quality healthcare, programs, services, and benefits to support you on your path to wellness.

- How easy is it to get an appointment with your doctor or fill a prescription at your pharmacy?
- As your health plan, does our Member Services team help answer your questions?
- How do you rate us as your health plan on a scale of 0-10?
And what will we do with your feedback?
As your health plan, we will keep working to make sure we meet your needs by:
- Working with providers to help you get appointments when you need them
- Answering your health plan questions with a Member Services team and personal Health Care Buddy who is there for you
- Supporting you with physical and mental health and wellness programs
