The Outreach team strives to bring awareness of our health plan by reaching across Arizona to connect those in need with health insurance. We are passionate about building diverse relationships with our community partners, members, and providers.

At BCBSAZ Health Choice Pathway (HMO D-SNP), we value member voice and choice. What does that mean? It means all members have the right to voice their opinions and choose how they receive healthcare services to meet their needs.

The committee’s purpose is to get feedback from our members – like yourself! You can make a difference by sharing your experiences as a member of our health plan and help us make improvements.

Community Resources

Rental Assistance Is Available To Help Renters and Landlords

Renters and landlords in need of financial assistance can use a new website, consumerfinance.gov/renthelp, from the Consumer Financial Protection Bureau to find rental assistance providers in your area.

If you know of a renter who’s having trouble paying rent, utilities, or other housing costs – or a landlord trying to stay afloat with tenants in this situation – help may be available. State and local programs are distributing billions of dollars in rental assistance to help renters stay housed during the pandemic.

Visit the CFPB’s Rental Assistance Finder to find resources for renters and landlords to help navigate various financial hardships related to the pandemic.
